Iridium GO! - Display reads 'Enter PUK code'

Power on the Iridium GO!.
Join the Iridium GO! Wi-Fi network using your mobile device.
Open the Iridium GO! application
When prompted, enter username: guest and password: guest.
Select the Submit button.
A "PUK code required" pop-up will be displayed.
Enter the PUK code in the Enter PUK code field.
Enter the SIM PIN 1111 in the Enter SIM PIN field. This will reset to the default PIN.
Select the OK button.
